Once or twice I've cut up a Word file with this problem into chunks and then imported them separately in hopes of avoiding cut-and-pasting a long series of notes. I'm not sure this is worth the extra attention required to make sure the pieces integrate properly; if nothing else, the repeated cut-and-paste at least minimizes mental effort. Ken's solution is more clever, and I'd love to learn whether it works.
